Adolescent Health Program

Southern Seven Health Department's Southern Illinois Adolescent Health Program aims to empower adolescents, ages 11-21, to adopt healthy behaviors and improve their overall health by increasing the rate of adolescent well-care visits. For the purposes of this program, a well-care visit is defined as a comprehensive visit with a health provider, occurring at least once a year, in which a physical, emotional, and social development is assessed in alignment with the American Academy of Pediatrics Bright Futures Guidelines. The program is funded in whole or in part by by the Illinois Department of Public Health.

A yearly well-care visit can improve overall health!

This well-care appointment focuses on keeping adolescents, ages 11-21, healthy by addressing health risks before they are a big concern. A well-care visit includes the following:

  • Physical exam, height/weight and blood pressure checked.
  • Check for behavioral and mental health concerns.
  • Advice and support on staying healthy (eating habits, stress management, relationships, physical and social activity, sexual health).
  • Immunizations/vaccines as needed.
